NOTE: All Fine Arts participants must also fill out the Individual Registration Form and send it to the Oregon Ministry Network by March 11. All entries for ART Division Categories must be sent to Valley Life Center Attn: Jerry Mullins, 1795 SE Miller AVE Dallas, OR 97338 by March 11. All entries for WRITING Division Categories must be sent to OMN Attn: Amplify, P.O. Box 9178 Salem, OR by March 11. Entries received after the postmark date of March 11, 2010 will not be accepted.

Pre Student Pass (By Mar. 11) - $45
Late Student Pass** (After Mar. 11) - $55
Paid Leader Event Pass**** ($35)
Additional Entry*** (By Mar. 11) - $5
*The Student /Leader Pass is for Thursday, Friday and Saturday and includes the first two Fine Arts Category entries. This is a non-refundable fee.
**Late Registration is only available for non-participants not performing in Fine Arts categories.
***Each additional Fine Arts entry after the initial 2 entries will cost $5.00 per entry, postmarked by the deadline of March 11, 2010. This is a non-refundable fee.
****Two youth leader passes are provided free of charge for the supervision of the first 16 students. For every grouping of 8 students after the first 16, one free pass will be issued to a youth leader over the age of 18.
A senior pastor and his/her spouse may receive a free pass by requesting them by phone at least two weeks prior to the event. This pass is not to be used by a student OR youth leader.
